Moving to Pitman Manor

  1. Fill out the application – The application requires basic information along with financial information. This application is used as the basis for determining financial eligibility.

  2. Once you have received financial approval, you will be asked to provide medical information. This will be used by our nursing staff during your assessment to determine what, if any, level of care is needed upon move-in. You will also need to gather copies of documents so a checklist is attached for your convenience.

  3. Return all your medical paperwork, along with copies of all documents outlined in the Checklist for New Residents to the marketing director before your assessment is scheduled.

  4. Schedule and complete your assessment with the Director of Wellness. The Marketing Director can coordinate this for you.

  5. Move In!
